Antecedentes y objetivo Evaluar el impacto económico en España derivado de la introducción del implante XEN®63 como alternativa quirúrgica en el manejo del glaucoma primario de ángulo abierto (GPAA) con o sin cataratas en pacientes refractarios. Materiales y métodos Se diseñó un análisis de impacto presupuestario para estimar los costes del tratamiento quirúrgico del GPAA desde la perspectiva del Sistema Nacional de Salud (SNS) español, en un horizonte temporal de1 año. Los comparadores considerados (trabeculectomía, esclerectomía profunda no perforante, válvula de Ahmed, iStent inject®, Preserflo® microshunt y XEN®45) se corresponden a los empleados en la práctica clínica pública española. Para el cálculo de la población diana, cuotas de mercado y uso de recursos en términos de visitas de seguimiento, procedimientos adicionales y complicaciones poscirugía, se emplearon datos de la literatura y se validaron con un panel de cuatro expertos. Los costes unitarios (€de2021) se obtuvieron de la base de datos ESALUD Resultados la inclusión de XEN®63 podría generar un ahorro de 2.569.737€ tras un año desde su introducción, derivado del ahorro en el coste del implante y del procedimiento (−423.120; −0,7%), de las visitas de seguimiento (−777.407€; −4,5%), de los procedimientos adicionales (−1.048.145€, −20,6%) y de las complicaciones poscirugía (−321.065€, −14,2%). Conclusiones La incorporación de XEN®63 en el arsenal quirúrgico para el tratamiento del GPAA refractario con y sin cataratas podría generar ahorros para el SNS (AU)

The natural armors and weapons of the animal kingdom are serving as inspiration in the development of next-generation engineering materials. In this pursuit, seldom considered are the variations in properties across taxa that have evolved to meet their unique functional demands. Here, teeth from six different mammalian species were acquired and categorized according to their bite force quotient (BFQ), which accounts for the allometric scaling between bite force and body size. Selected chemical, microstructural, and mechanical properties of the enamel were quantified across the enamel thickness using spectroscopy and indentation techniques. Results showed that the chemical composition of enamel was significantly (P <  0.05) different between the Low and High BFQ groups, whereas the apatite crystallinity was not. The enamel of all animals exhibited a spatial gradient in mechanical properties that was consistent when evaluated using a normalized framework. Although the elastic modulus, hardness and indentation brittleness were significantly lower in the High BFQ group, the fracture resistance of enamel was significantly higher in this group, potentially reflective of bite force requirements related to diet and predation. Enamel rod decussation was present in all teeth, but there were differences in specific microstructural features. Overall, these results highlight that the diversity of tooth enamel across species should be considered in the pursuit of nature-inspired structural materials. STATEMENT OF SIGNIFICANCE: Natural weapons are serving as inspiration in the development of next-generation engineering materials. Tooth enamel is a viable candidate, but variations in the structure and properties of enamel across taxa have not been explored. Here, teeth from six different mammalian species were categorized according to their bite force quotient (BFQ), and the enamel was compared in terms of selected chemical, microstructural, and mechanical properties. We show that specific aspects of the chemical composition and properties of the Low and High BFQ groups are unique, which appears reflective of bite forces associated with diet and predation. Overall, the results highlight that the diversity of tooth enamel across species should be considered in the pursuit of nature-inspired structural materials.

RESUMEN Los polimorfismos genéticos asociados con las caseínas de la leche son de gran importancia, ya que pueden ser usados como marcadores genéticos para mejorar el rendimiento productivo en los hatos lecheros. El objetivo de este estudio fue evaluar la diversidad y estructura genética de 5 SNP de caseínas de la leche, obtenidos con chips genómicos en vacas y toros de raza Holstein en Antioquia (Colombia). Fueron muestreados 113 animales de raza Holstein en 3 regiones del departamento de Antioquia (norte, centro y oriente) y un cuarto grupo de sementales comerciales. Los animales fueron genotipificados con chips genómicos de alta densidad (Illumina BovineHD e Illumina SNP50 v2), a partir de los cuales se identificaron 5 SNP (ARS-BFGL-NGS-8140, BTA-77380-no-rs, BTA-32346-no-rs, BTB-00821654 y ARS-BFGL-NGS-15809). Para cada SNP se realizó un análisis genético mediante un análisis de varianza molecular (amova) usando el software GenAIEx 6.501. Los SNP con mayor heterocigosidad total (HT) fueron ARS-BFGL-NGS-8140 y BTA-32346-no-rs, con resultados cercanos al 45%; sin embargo, la Ht para ARS-BFGL-NGS-15809, BTA-77380-no-rs y BTB-00821654 estuvo por debajo del 15%. El SNP con mayor diversidad genética fue BTA-32346-no-rs (Ho-He = 0,06; p < 0,05). En esta investigación se evaluó una subpoblación de toros comerciales extranjeros, en la cual se obtuvieron frecuencias alélicas y genotípicas similares a las obtenidas para las subpoblaciones locales, sugiriendo que los alelos de los toros muy posiblemente están fijados en dichas subpoblaciones, por lo que la estructura y diversidad genética tienden a ser bajas en la muestra de estudio.


ABSTRACT Genetic polymorphisms associated with milk caseins have a great importance since they can be used as genetic markers to improve productive performance in dairy herds. The main goal of the present study was to evaluate the diversity and genetic structure of 5 SNPs of milk caseins, obtained with genomic chip in Holstein cows and bulls from Antioquia (Colombia). 113 Holstein animals were sampled in 3 regions of Antioquia (north, center, and east), and a fourth group of commercial sires. Animals were geno-typed with high-density SNP chips (Illumina BovineHD and Illumina SNP50 v2), from which 5 SNPs were identified (ARS-BFGL-NGS-8140, BTA-77380-no-rs, BTA-32346-no-rs, BTB-00821654 and ARS-BFGL-NGS-15809). For each SNP, a genetic analysis was performed by means of an analysis of molecular variance (AMOVA) using the GenAIEx 6.501 software. The SNPs with the highest total heterozygosity (Ht) were ARS-BFGL-NGS-8140 and BTA-32346-no-rs, with results close to 45%; however, the HT for ARS-BFGL-NGS-15809, BTA-77380-no-rs, and BTB-00821654 were below 15%. The SNP with the highest genetic diversity was BTA-32346-no-rs (Ho-He = 0,06; p < 0,05). In this research a subpopulation of foreign commercial bulls was evaluated, in which similar allelic and genotypic frequencies to those for local subpopulations were obtained, suggesting that the alleles of the bulls are very possibly fixed in these subpopulations, so that the structure and genetic diversity tend to be low in the study sample.

Objetivos Evaluar los resultados de una nueva modificación de la esclerectomía profunda no perforante (EPNP) denominada espolonectomía. Métodos Estudio retrospectivo no aleatorizado y comparativo que incluyó 98 ojos (grupoA) operados con espolonectomía, que consiste en la combinación de la escisión del espolón escleral con EPNP. El grupo control (B) incluyó 53 ojos operados con EPNP clásica. Durante 12meses se estudiaron la presión intraocular (PIO), los medicamentos antiglaucomatosos que requirieron y las complicaciones. Resultados La PIO media disminuyó de 25,69±8,11 preoperatoria a 15,73±4,16mmHg después de la cirugía en el grupoA (p<0,001). En el grupoB disminuyó de 26,66±5,93 preoperatoria a 18,19±5,93mmHg postoperatoria (p<0,001). Las diferencias entre grupos de la PIO postoperatoria fue estadísticamente significativa (p<0,001). A los 12meses después de la cirugía, el 13,27 y el 52,83% de ojos en los gruposA yB requirieron tratamiento antihipertensivo tópico (p<0,001). La tasa de éxito absoluto después de la cirugía fue del 87,5 y del 47,17% en los gruposA yB, con una tasa significativamente mayor de éxito relativo en el grupoB (p<0,001). No hubo diferencias significativas entre los grupos en la tasa de complicaciones (p=0,960). La complicación postoperatoria más frecuente fue la microperforación de la membrana trabeculo-descemética en ambos grupos. Conclusiones La espolonectomía es una técnica segura y eficaz en comparación con la EPNP convencional y parece una alternativa prometedora en el tratamiento quirúrgico del glaucoma, optimizando la eficacia del tratamiento y minimizando las complicaciones (AU)

OBJECTIVE: To review the clinical characteristics and complications associated with cholelithiasis in pediatric patients by identifying risk factors and assessing surgical management and results. METHODS: Retrospective study in patients under 18 years of age undergoing cholelithiasis surgery. The following data were analyzed: age, sex, body mass index (BMI), associated comorbidities, clinical presentation, symptom duration, surgical treatment, pathological report, postoperative complications, and hospital stay. RESULTS: 135 cholelithiasis patients underwent surgery from 2013 to 2018, with an increasing trend in the annual number of cholecystectomies. Most patients were adolescents (86.7%) and female (72.6%), and they had cholesterol gallstones (86.6%) and >85 BMI (33%). Mean symptom duration was 85 days (SD: 148). Symptomatic cholelithiasis was present in 131 cases (97%). CL associated complications were recorded in 64.4% of patients, with cholecystitis, pancreatitis, and choledocholithiasis being the most frequent ones. All patients underwent laparoscopic cholecystectomy; 4 (2.9%) required conversion to open surgery, and 6 (4.4%) had postoperative complications. Mean hospital stay and postoperative follow-up were 5.7 days (SD: 4) and 2.3 months (SD: 1.9), respectively. CONCLUSIONS: Non-hemolytic cholelithiasis in the pediatric population is more frequent in female overweight and obese adolescents. Symptomatic cholelithiasis is associated with higher risk of complications such as cholecystitis, pancreatitis, or choledocholithiasis, which supports early surgical management.


OBJETIVO: Revisión de las características clínicas y complicaciones asociadas de los pacientes pediátricos con colelitiasis, mediante la identificación de factores de riesgo, la evaluación del manejo quirúrgico y sus resultados. METODOS: Estudio retrospectivo en pacientes menores de 18 años, intervenidos quirúrgicamente por colelitiasis. Se analizaron datos como edad, género, índice de masa corporal (IMC), comorbilidades asociadas, presentación clínica, tiempo de los síntomas, tratamiento quirúrgico, reporte de patología, complicaciones posoperatorias y estancia hospitalaria. RESULTADOS: Se intervinieron quirúrgicamente 135 pacientes con colelitiasis entre los años 2013 a 2018, con una tendencia ascendente en el número de colecistectomías realizadas anualmente. La mayoría fueron pacientes adolescentes (86,7%), con cálculos de colesterol (86,6%), de género femenino (72,6%) y con IMC > 85 (33%). El tiempo de los síntomas en promedio fue de 85 días (DE 148). Colelitiasis sintomática se presentó en 131 casos (97%). Complicaciones asociadas a CL se registraron en 64,4% pacientes, siendo las más frecuentes colecistitis, pancreatitis y coledocolitiasis. En todos los pacientes se realizó colecistectomía laparoscópica, 4 (2,9%) requirieron conversión a cirugía abierta y 6 (4,4%) presentaron complicaciones postoperatorias. La estancia hospitalaria y el seguimiento posoperatorio fueron en promedio 5,7 días (DE 4) y 2,3 meses (DE 1,9), respectivamentea. CONCLUSIONES: La colelitiasis no hemolítica en la población pediátrica, es más frecuente en pacientes adolescentes de género femenino con sobrepeso u obesidad. La colelitiasis sintomática está asociada con mayor riesgo de complicaciones como colecistitis, pancreatitis o coledocolitiasis, en consecuencia, se justifica una intervención quirúrgica temprana.

Dentin undergoes irreversible changes in microstructure with aging that involve gradual filling of the tubule lumens with mineral. Known as dental sclerosis, this process begins at the root apex, progresses coronally, and is associated with a degradation in the resistance to fracture of dentin. OBJECTIVE: To determine i) age-related changes of intertubular dentin with aging, particularly within the root, and ii) the differences in age-related degradation between vital and pulpless (i.e. non-vital) teeth. METHODS: We performed nanoscopic dynamic mechanical analysis (nanoDMA) in scanning mode on the intertubular and peritubular dentin of teeth from young and old adults. The complex, loss and storage moduli, as well as the tan delta parameter were evaluated for teeth with no restorations and teeth with root canal treatment (non-vital). RESULTS: There were significant changes in the dynamic moduli of intertubular dentin with age, which were most substantial in the apical third of the root. The storage modulus of the intertubular dentin, which quantifies the purely elastic resistance to deformation, was significantly (p < 0.0005) larger for both the old vital and non-vital teeth than that of the young teeth, over the entire root length. However, the tan delta parameter, which quantifies the relative capacity for viscous deformation, was significantly lower in these two groups (p < 0.005). SIGNIFICANCE: Radicular dentin undergoes an embrittlement with aging, involving reduced capacity for viscous deformation. The extent of degradation is largest in the apical third. Removal of the pulp appears to accelerate the aging process or compound the extent of degradation.

RESUMO

Presentamos un caso de una paciente de 3 años de edad con diagnóstico de retinoblastoma en el ojo izquierdo. El curso de la enfermedad obligó a la enucleación de dicho ojo e implantación de una prótesis. Dos años tras la cirugía, se produjo una extrusión parcial de la prótesis en la cavidad anoftálmica y se decidió emplear un sustituto semisintético de duramadre para cubrir el defecto. El uso de un sustituto semisintético de duramadre en este escenario específico no ha sido comunicado previamente

Two years after surgery, partial prosthesis extrusion occurred and a semisynthetic dura mater substitute was used as a patch graft to cover the defect. To our knowledge, semisynthetic dura mater substitutes' use in this scenario has not been previously reported.

RESUMO

El ángulo iridocorneal por sus implicaciones en la fisiopatología del drenaje del humor acuoso es una estructura fundamental de la cámara anterior. La tomografía de coherencia óptica de segmento anterior es una técnica rápida y no invasiva que obtiene imágenes de los tejidos vivos con una alta resolución permitiendo conocer la anatomía normal del ángulo, sus alteraciones y los cambios que se producen en el mismo tras diferentes intervenciones terapéuticas. La tecnología de la tomografía de coherencia óptica de segmento anterior ha ido evolucionando hasta ofrecer imágenes que permiten identificar y cuantificar estructuras angulares claves en sujetos sanos y en pacientes con glaucoma, especialmente la malla trabecular y el canal de Schlemm, lo que puede contribuir a ampliar el conocimiento de la fisiopatología del glaucoma. Además, permite cuantificar la abertura angular con unos parámetros objetivos descritos en los últimos años, entre los que destacan el ángulo iridotrabecular, la distancia de abertura angular y el área iridotrabecular. La tomografía de coherencia óptica de segmento anterior presenta múltiples utilidades en el estudio de los distintos mecanismos del cierre angular, la evaluación de los cambios angulares tras la realización de una iridotomía láser o iridoplastia, cirugía de la catarata o el implante de lentes fáquicas


The iridocorneal angle, due to its implications in the physiopathology of aqueous humour drainage, is a fundamental structure of the anterior chamber. Anterior segment optical coherence tomography is a rapid and non-invasive technique that obtains images in vivo. The high resolution allows it to analyse the normal anatomy of the angle, any alterations, and the changes that occur after different therapeutic interventions. Anterior segment optical coherence tomography technology has evolved to provide images that allow the identification and quantification of the angular structures in healthy subjects and in glaucoma patients, and especially the trabecular meshwork and the Schlemm's canal. It also enables the angle width to be quantified, with some objective parameters that have been standardised in recent years, such as the trabecular-iris angle, the angle opening distance, and the trabecular-iris area. This technique has multiple uses in the study of the different mechanisms of angle closure, the evaluation of changes after a laser peripheral iridotomy or iridoplasty after cataract surgery, as well as after the implantation of phakic lenses

Objetivos: Conocer la incidencia de obstrucción aislada de la arteria ciliorretiniana (OACR) en nuestra serie, a lo largo de 5 años, y describir la incidencia, evolución y secuelas oftalmológicas de esta entidad en el embarazo. Métodos: Estudio retrospectivo, descriptivo, observacional de serie de casos. Resultados: De los 135 pacientes diagnosticados de oclusión arterial retiniana del total de nuestra serie, 20 (14,8%) presentaban OACR, de los cuales 2 (1,48%) eran mujeres embarazadas. Caso 1: mujer embarazada de 34 años, con escotoma centrocecal y agudeza visual de 1 en ojo derecho. Fondo de ojo: exudado algodonoso en el haz papilomacular con edema retiniano y émbolos en una de las ramas de la arteria ciliorretiniana. A las 5 semanas el exudado y el edema habían desaparecido, con reducción del escotoma. Caso 2: mujer embarazada de 30 años, que presenta de forma brusca escotoma centrocecal en el ojo derecho con agudeza visual de 1. Fondo de ojo: exudación lipídica y mancha algodonosa en haz interpapilomacular secundarias a la OACR. A las 4 semanas habían desaparecido los hallazgos en fondo de ojo, con resolución completa de la sintomatología. Conclusiones: La etiología de la OACR suele estar relacionada con enfermedad carotídea u otros procesos tromboembólicos relacionados con estados de hipercoagulabilidad y autoinmunidad. El embarazo es considerado un estado de hipercoagulación, sin embargo, no se ha podido demostrar que se trate de un factor de riesgo per se para desarrollar embolia arterial. Por tanto, se necesitan estudios adicionales para conocer la correlación entre embarazo y OACR (AU)

The exudate and oedema disappeared after 5 weeks, and the scotoma was reduced. Case 2: A 30 year-old pregnant woman, with normal visual acuity in right eye, and a centrocaecal scotoma. Fundoscopy: An area of retinal interpapillomacular infarction due to cilioretinal artery occlusion. The fundus returned to normal in 4 weeks, with an improvement of the scotoma. Conclusions: The aetiology of CRAO is usually associated with carotid disease or other thromboembolic events related to hypercoagulable states and autoimmunity. Pregnancy is considered a hypercoagulable state, and it is not known if it is a risk factor for arterial embolism. Further studies are required to determine the correlation between pregnancy and CRAO (AU)
